We are currently seeking a dedicated and experienced MDS Coordinator to join our team at Elkader Care Center located in Elkader, IA. The MDS Coordinator is a vital member of our interdisciplinary team, responsible for assessing residents and developing care plans to ensure they receive the highest quality care.
Responsibilities:
- Coordinate the development and completion of the MDS for each resident
- Collaborate with healthcare providers to develop comprehensive care plans
- Ensure accurate and timely documentation of resident assessments and care plans
- Maintain compliance with state and federal regulations regarding MDS assessments
- Participate in interdisciplinary meetings to discuss resident progress and adjust care plans as needed
Qualifications:
- Active RN license in the state of Iowa
- Prefer 2 years experience as an MDS Coordinator in a long-term care setting
- Strong attention to detail and organizational skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
